 If you are unable to fill your assignment(s) due to vacation or illness, or some other reason, please inform the contact manager, or Jeff & PK.  You may also make arrangements with another LEADer to trade with you, or fill in during your absence.  Be sure to let us know about your alternate arrangements.

Steps for Successful Application

1.      Fully complete the application form.

2.      Write the essay described in the application form and attach it to the application.

3.      Attach a recent photo of yourself to the application form.

4.      Have your parent/guardian fill out and sign the parent information sheet and parent evaluation.

5.      Contact your chosen references and ask them to complete one of your reference forms.  A teacher or other responsible leader should complete at least one of your references.  References may be mailed by the reference or may be returned by the applicant in a sealed envelope.  YOUR APPLICATION IS NOT COMPLETE WITHOUT THESE REFERENCES.

6.      Submit your application (along with the Parent Information Sheet and completed Reference Forms) to Jeff and PK McCann using the information on the Contact Us Page.

7.      Schedule and complete an oral interview.  Interviews are by appointment only.  You will be contacted to set an appointment time once your application is complete.

Only after all these steps have been completed can you be notified about acceptance into the program.  Once the application process is completed, you will be notified of your status promptly.

Instructions:

1.      Please complete the sermon outline from the Church bulletin as you follow along during the service.

2.      Also, answer the questions on the Sermon Feedback Form.  If needed, you may continue your answers on the back of the page

3.      Turn in the sermon outline along with the Feedback form.

4.      Be prepared to discuss your answers.

Instructions for Evaluators:

1.      Please observe the LEAD Trainee with appropriate discretion.  Refrain from interrupting, and “coaching”, unless your assistance is requested.

2.      If you are a Teacher or Leader who is being assisted by a LEAD Trainee, please provide them opportunity to display and utilize the skills they have been learning.

3.      Complete the Evaluation Form by providing scores and comments in all Skill areas which would be appropriate to your observation.  (Scores are rated from 1 to 10, with 10 being the highest or best use of a skill)

4.      Also provide scores and comments in the Attitude areas which apply to your experience or observation of the trainee.  (Scores are rated from 1 to 10, with 10 being the highest or best display of appropriate attitude)

5.      Remember that comments should be specific and helpful in improving performance of a skill or display of an appropriate attitude.  “Slams” or “put-down statements” are not appropriate Evaluation comments.  Praise comments are welcome, but should also be specific and helpful in determining what areas of the trainees’ performance are going well.
